As a nurse in complex care packages, you will be responsible for providing high-level, hands-on clinical care tailored to the needs of individuals with multiple and often complex medical conditions. You will need to exercise expert clinical judgment, coordinate care across multidisciplinary teams, and provide continuous support to both Service users and their families, ensuring they receive the best possible care in the community.

What will you do?

  • Comprehensive Service user Assessment

    Initial Assessment: Conduct thorough assessments of the patient’s physical, emotional, and psychological needs, including reviewing their medical history, current conditions, and treatment requirements.

    Ongoing Monitoring: Regularly monitor Service users' vital signs, health status, and progress, adjusting care plans as needed. This includes assessing for any deterioration or complications, such as pressure ulcers, infections, or changes in cognitive or respiratory function.

    Holistic Care Planning: Collaborate with Service users, families, and multidisciplinary teams to develop and update personalised care plans tailored to the individual's complex health conditions.

  • Clinical Interventions and Care Delivery

    Medication Management: Administer prescribed medications, ensuring safe, accurate dosage and timing, and manage complex medication regimens (e.g., injections, IV infusions, controlled substances).

    Wound Care: Provide advanced wound care for chronic wounds, pressure ulcers, or surgical sites, including dressing changes, monitoring for infection, and documenting progress.

    Respiratory Support: Manage and monitor respiratory care, including tracheostomy care, oxygen therapy, nebulisation, CPAP/BiPAP machines, or ventilation, ensuring safe and effective delivery.

    Catheter Care: Insert and manage urinary catheters, including indwelling catheters, and monitor for signs of infection or complications.

    Gastrostomy and PEG Tube Care: Administer enteral feeding, monitor for complications such as tube dislodgement or infections, and ensure nutritional needs are met.

    Pain Management: Assess and manage pain, providing appropriate interventions and adjusting care plans based on the patient's comfort levels and clinical needs.

    End-of-Life Care: Provide palliative and hospice care, including symptom management, comfort care, and emotional support for both the Service user and their family members, when necessary.

  • Clinical Monitoring and Decision-Making

    Continuous Monitoring: Use equipment to monitor vital signs (blood pressure, heart rate, respiratory rate, oxygen saturation, etc.) and identify any significant changes or abnormalities that require urgent intervention.

    Clinical Judgement: Make independent, evidence-based decisions regarding Service user care, such as adjusting medications, recommending further diagnostics, or escalating care needs.

    Emergency Response: Be prepared to respond to emergencies, such as sudden deterioration, seizures, cardiac arrest, or respiratory failure, using clinical protocols and advanced life support measures.

  • Care Coordination and Multi-Disciplinary Collaboration

    Liaison with Clinical Lead: Communicate regularly with your Clinical Lead for the package of care to pass on relevant information for MDT meetings.

    Case Reviews: Participate in multidisciplinary case reviews, discussing Service user progress, challenges, and care plan adjustments, ensuring holistic and integrated care delivery.

    Advocacy: Advocate for Service users to ensure they receive the appropriate resources, services, and support to meet their care needs.


  • Education and Support

    Service user and Family Education: Educate Service users and their families about managing their conditions, the correct use of medical equipment (e.g., ventilators, catheters, feeding tubes), and signs of complications or
    deterioration.

    Training and Support: Offer training and guidance HCA’s if required on how to manage daily care tasks safely and effectively, empowering them to participate in the patient’s care.

    Health Promotion: Educate on lifestyle changes, infection prevention, nutrition, and other relevant health topics that can improve the patient’s quality of life.


  • Documentation and Record-Keeping

    Accurate Record Keeping: Maintain comprehensive and up-to-date Service user records, documenting all assessments, interventions, changes in condition, and communication with other healthcare providers.

    Legal and Ethical Compliance: Ensure all documentation and care delivery meet legal, ethical, and professional standards, ensuring that the patient’s rights, safety, and dignity are upheld.

    Reporting: Regularly report any significant changes or concerns about the patient’s health to senior clinical staff or other members of the multidisciplinary team.

  • Quality Assurance and Risk Management

    Care Standards: Ensure care is delivered in line with relevant clinical guidelines, policies, and best practices to provide safe and effective care.

    Infection Control: Implement and follow infection prevention and control measures to minimise the risk of hospital-acquired infections, particularly in Service users with complex care needs.

    Risk Management: Identify potential risks to Service user safety, such as medication errors, falls, or pressure sores, and take proactive steps to mitigate those risks.

    Dignity and Respect: Ensure Service users are treated with dignity and respect, particularly when dealing with challenging or sensitive care needs.

Essential Qualifications and Skills:

  • Excellent Clinical Judgement: Ability to assess, diagnose, and make informed decisions based on Service user needs.

  • Strong Communication Skills: Capable of liaising with Service users, families, and multidisciplinary teams effectively.

  • Compassion and Empathy: A genuine commitment to delivering person-centred care with sensitivity to the emotional and physical needs of Service users.

  • Time Management and Organisation: Ability to manage a varied caseload and prioritise tasks in a community setting.
